Player Eligibility Roster - required, used to submit the eligible players for a team (Player Eligibility Roster in Microsoft Word format)
Team Data Sheet - required, used to submit information on the school, coaches, gym location, etc. for a team (Team Data Sheet in Microsoft Word format)

Accidents, Emergencies, Injury Reporting

Accident-Emergency Information - keep nearby with vital information that may be needed in case of an accident or emergency
Athletes Emergency Information - keep nearby with emergency information for individual players
Injury Reporting - used to report an injury to the Diocese

Coaching Clearances and Pledge

Diocese of Pittsburgh Centralized Database - required, to help ensure a safe environment for young people, the Diocese of Pittsburgh has created a centralized database of those that work and minister in the Church who have contact with young people under the age of 18. All who have contact with the youth of the Dioceses must fill out the online application. This includes any clergy, religious employee or volunteer (including coaches). Section Schedule Spreadsheet

Only section games should be included on the spreadsheet. The individual coaches can add scrimmages and tournament games for their team on the web site on their team’s page (once they log in). Those games will appear on the team’s page but not on the section schedule.
